Medisafe App Review (2026): Features, Medfriend, Price and Who It Suits

What the Medisafe app does, how its Medfriend family alerts really work, what is free in the US versus Premium, and whether it fits a family looking after an elderly parent.

The Medisafe app is a pill reminder and medication tracker made by MediSafe Inc. In the US it is free to use, with an optional Premium subscription, and it can alert a family member (a “Medfriend”) about an hour after a dose is missed. This review looks at how it works for families who help an older parent with their medicines.

We did not test Medisafe hands-on for this review. Everything below comes from Medisafe’s own help center, website and app store listings, which we checked on September 30, 2026 and link in the sources. We do not give Medisafe a score or star rating.

What is the Medisafe app?

Medisafe is a medication management app for iPhone and Android. The App Store lists the seller as MediSafe Inc., and the app has more than 5 million downloads on Google Play. Medisafe’s help center describes it as a cloud-synced medication management platform that lets family, friends and caregivers be alerted about whether you took your medicine.

One thing worth knowing: Medisafe’s main website now presents the company as a “patient engagement engine for pharma portfolios,” working with more than 25 pharmaceutical partners. The consumer Medisafe medication management app is one part of that business.

How the Medisafe pill reminder app works

You add each medicine (picked from a drug list or typed in by hand), set its schedule, and the app reminds you when a dose is due. The Google Play listing says it supports complex schedules, “as needed” medicines, vitamins and supplements, a weekend mode for sleeping in, and automatic time zone detection.

Reminders, snooze and follow-ups

  • Repeat reminders on iPhone. Medisafe’s help center says that on iOS, if you do nothing with a reminder, it repeats every 10 minutes by default.
  • Custom snooze. On both iPhone and Android you can change the snooze length. On iPhone the custom length only applies when you actually tap “Snooze.”
  • Snooze until home or work. If you allow location access and save your addresses, you can snooze a dose until you arrive at home or work.
  • Mark from the notification. You can log a dose straight from the reminder without opening the app. The App Store listing also mentions a Lock Screen widget.

Dose log and reports

Each dose you mark builds a history. The home screen shows the last 30 days and the next 30 days. For anything older, the Reports section lets you email a report of up to one year, filtered by medicine if you like. The App Store listing describes this as a PDF you can send to a doctor or nurse.

Refill reminders

You enter how many pills are on hand and choose when to be warned, such as “when I have 20 left.” The app counts down as doses are marked. Medisafe is clear that it does not order or ship refills. You still call the pharmacy or doctor.

Health trackers and appointments

Health trackers let you log measurements such as blood pressure, blood sugar and weight, with optional reminders. Some trackers sync with Apple Health. According to the Google Play listing, access to more than 20 health measurements is part of Premium. There is also an appointment list that can sync with Apple or Google Calendar.

Drug interaction checker

Medisafe shows possible drug-to-drug interactions in an “Interactions” tab for each medicine. The help center says this only works when the app is set to English, only for medicines added from its drug list, and the Play listing says it is US only. Medisafe does not list it as a Premium feature. Medisafe itself says it does not provide medical advice and recommends asking a professional about interactions.

Medfriend: how Medisafe alerts family about a missed dose

A Medfriend is a family member or friend who links their own Medisafe app to your parent’s account. It is the feature most adult children care about, so here is exactly how Medisafe’s help center describes it:

  • Who is notified: the Medfriend, on their own phone, through the Medisafe app. They join by entering a 4-digit code from an invite sent by text or email.
  • When: only if no action is taken on the dose reminder. The alert goes out 30 minutes after the last reminder, which is about 1 hour after the scheduled time.
  • Can it be changed? No. The interval is fixed for now. Rescheduling a dose resets the timer.
  • What a Medfriend can do: view medicines and doses only. They cannot mark a dose or change a medicine. Medisafe says this is for security.
  • Dependents: Medfriends only get missed-dose alerts for the main profile on an account, not for dependent profiles, though they can still view them.
  • Cost: Medisafe’s download page calls adding a Medfriend free. Premium adds unlimited Medfriends, so the free version has a limit. Medisafe does not publish the exact number.

The help center also admits a sync issue: a Medfriend can sometimes get an alert for a dose that was marked taken, usually when the app is closed in the background. Managing a parent’s medications: Medfriend or dependent profile?

Medisafe offers two setups for caregivers. Which one fits depends on who marks the doses.

Sharing one login has trade-offs. Medisafe’s own time zone article says that if family members live in different time zones, the main account should stay on the device in the patient’s time zone and others should use Medfriend instead.

Is Medisafe free? Free vs Premium and price

In the US, Medisafe is free to download and use, and Premium is optional. That is stated in Medisafe’s Google Play listing, updated September 15, 2026. Medisafe’s help center lists what Premium adds:

  • Unlimited medications
  • Unlimited Medfriends
  • Unlimited dependents
  • Health trackers (the Play listing says more than 20 measurements)
  • All “Medtones” reminder sounds and color themes

Medisafe does not publish the exact caps on the free version. If your parent takes many medicines, check the limit inside the app before you enter everything.

Medisafe Premium price: the help center lists $4.99 USD per month or $39.99 USD per year (as of September 2026). The US App Store shows several Premium price points, from $2.99 to $9.99 a month and $29.95 to $39.99 a year, so the app may show you a different offer. You pay through the App Store or Google Play, it renews automatically, and Medisafe says insurance does not pay for it.

Outside the US

Since January 1, 2026, Medisafe requires a subscription for users outside the United States, after a 14-day free trial. If your parent lives abroad, this is the version they will get. Does Medisafe have ads? What about privacy?

Medisafe’s sources disagree here, so we report all three:

  • Medisafe’s help center says the app “is, and will remain, ad-free,” and that it never sells or shares personal information.
  • The Google Play listing carries Google’s “Contains ads” label.
  • The App Store privacy label says some usage and advertising data, not linked to your identity, may be used for “Third-Party Advertising.”

On security, the help center says all data is encrypted and that Medisafe is HIPAA compliant. The Play data safety section says no data is shared with third parties and that you can ask for your data to be deleted. If ads or data use matter to your family, read the current listing and privacy policy before you sign up.

Medisafe on Android, iPhone and Apple Watch

  • Medisafe app for Android: available on Google Play, with Wear OS support. The help center says Android watches may not show dependent or Medfriend profiles, but you can get reminders and mark doses.
  • iPhone: needs iOS 17 or later. In the US it can import medicines from Health Records on iPhone, where available.
  • Medisafe on Apple Watch: listed on the App Store (watchOS 10 or later). You turn it on in the Watch app on the iPhone.

Strengths

  • Free in the US, and Medisafe describes adding a Medfriend for missed-dose alerts as free.
  • Handles complex schedules, “as needed” medicines and weekend timing.
  • Refill countdown, appointment list and doctor-ready reports in one place.
  • Interaction checker for US users, as a prompt to ask the pharmacist.
  • Works on both iPhone and Android, plus Apple Watch and Wear OS.

Limitations for families of elderly parents

  • Alerts are slow and fixed. About an hour after the dose, with no way to make it sooner.
  • Your parent needs a smartphone and must tap “Taken.” Like any app, Medisafe records a tap, not a swallowed pill.
  • No alerts for dependent profiles. If you manage the medicines under your account, siblings cannot get missed-dose alerts as Medfriends.
  • Sharing means sharing a password for dependent profiles.
  • Free limits are not published, and the Medfriend view is read-only.
  • Sync can lag. Medisafe’s own troubleshooting articles mention false Medfriend alerts when the app is closed.
  • Mixed signals on ads between its help center and store labels.

Keep a paper backup either way. A printable medication list on the fridge helps if the phone is lost or the app stops syncing.

Who the Medisafe app is best for (and not for)

Good fit: a US-based parent who is comfortable with a smartphone, marks their own doses, and has one or two adult children who want a quiet heads-up when a dose is missed. For that family, the Medisafe app does a lot for free.

Poor fit: a parent who cannot use a phone reliably, families outside the US who do not want a subscription, or caregivers who need alerts sooner than an hour. For the bigger picture, see our guide to medication management for elderly parents.

DoseAnchor is our app. It is in development for iOS and Android and not available yet. It is built around the same worry Medfriend addresses: knowing a parent’s dose was taken without calling every day. Here is what we plan to offer at launch:

  • Free: 1 patient profile, up to 5 medicines, dose reminders with follow-ups and quiet hours, dose logging (taken, skipped or missed), pill count and refill reminders, appointment reminders, stats and a shareable health summary, a one-tap “I’m Okay” check-in, and 1 caregiver who can view the patient’s medicines and doses.
  • Premium ($4.99/month or $39/year): multiple patient profiles, unlimited medicines, up to 3 caregivers per patient, a missed-dose alert to the primary caregiver about 30 minutes after a dose goes unconfirmed (push and email), escalation to backup caregivers at about 60 minutes, low-stock emails, a daily summary email every evening, caregiver alert controls, a full refill dashboard, and CSV export of dose history.
  • Missed-dose alerts to family members are a Premium feature: the primary caregiver is alerted about 30 minutes after an unconfirmed dose, and up to 2 backup caregivers at about 60 minutes, by push notification and email. On the Free plan, 1 caregiver can view the dose log but does not receive missed-dose alerts. Caregivers never pay.

The honest limits: DoseAnchor will rely on your parent tapping “Taken,” so it cannot prove a pill was swallowed. Your parent will need a smartphone. It will not check drug interactions or give dosing advice, and it is not a medical device. See planned pricing or join the waitlist.

Frequently asked questions

Is the Medisafe app free?

In the United States, yes. Medisafe’s Google Play listing (updated September 2026) says the app is free to download and use in the US, with Medisafe Premium as an optional subscription. Outside the US, new users get a 14-day free trial and then need a subscription. Even in the US, unlimited medications, unlimited Medfriends and unlimited dependents are Premium features.

How much does Medisafe Premium cost?

Medisafe’s help center lists Premium at $4.99 USD per month or $39.99 USD per year (as of September 2026). The US App Store listing also shows other Premium price points, from $2.99 to $9.99 a month and $29.95 to $39.99 a year, so the offer you see may differ. You pay through the App Store or Google Play, and Medisafe says insurance does not cover it.

When does a Medisafe Medfriend get notified about a missed dose?

Only if nobody acts on the dose reminder. Medisafe’s help center says the Medfriend is notified 30 minutes after the last reminder, which is about 1 hour after the scheduled dose time. The timing cannot be changed, and rescheduling a dose resets the timer. Medfriends can view doses but cannot mark them or edit medicines.

Does Medisafe work on Apple Watch and Android watches?

Yes. The App Store lists Medisafe for iPhone and Apple Watch (watchOS 10 or later), and the Google Play listing says it is Wear OS enabled. Medisafe’s help center notes that a reminder shows on the phone or the watch, not both, and that Android watches may not show dependent or Medfriend profiles.

Can I manage my elderly parent’s medications in Medisafe?

Yes, in two ways. If your parent uses their own phone and marks doses, add yourself as a Medfriend to see their doses and get missed-dose alerts. If you handle the medicines for them, add a dependent profile under your account, which sends you every reminder. Medisafe says a dependent profile can only be shared by signing in to the same account.
